[Vimoutliner] Vorst dead end analysis

Hi David,

One thing I always do at the beginning of a project is a dead end analysis, so 
if there's something that's going to stop the show I find it before I've 
written 500 lines of code.

Here are some things I think are going to be difficult (not necessarily 
impossible, but difficult) in the VO to LaTeX converter:

Indexing
Bibliography
Labels and crossreferences
Math

We should devote some time to thinking how best to do these things.

The other thing I suggest is that you use LyX for awhile to make sure that 
doesn't scratch your itch. LyX is outstanding for book authoring. I've 
written an otl2lyx converter so the initial structure can be done in the 
(fast) VimOutliner, and then the authoring can be done in LyX. 

LyX recently acquired an "outline mode" so that chapters or sections can be 
created or moved around. It's obviously not as fast as VO, but the point is 
that on newer versions of LyX you DO have outlining capabilities.
